To determine the amount of exudate, use the following guideline by Richlen: None (no drainage) Dry dressing and wound bed Scant Nothing measurable on the dressing, wound bed moist Small/minimal 75% of the dressing involved, wound filled with fluid Wound assessment After a comprehensive patient assessment, evaluate the wound through visual inspection, palpation, and odor assessment
